If you are an existing patient, please first contact your pharmacy to check if a refill is on file. Your pharmacy may request a refill electronically. Or, you may complete a request in the form below.
Note: Refill requests must be sent 3 business business days (not including Saturday and Sunday) prior to running out/ expiring or a be subject to a $50 urgent refill fee. In order for the provider to approve a refill request, patients must have an upcoming appointment scheduled with the provider.

Please note: Emails will be read during normal business hours Monday-Friday. Your provider, or a member of the Psychiatry Management team, will responded as soon as possible during their schedule. Melissa Stefanowicz is in office Mondays-Thursdays.
If your request is urgent, and you need a response outside of regular office hours, a $50 fee will apply. CANCELLATIONS or RESCHEDULING: must be done with at least 24 hours notice or be subject to the cancellation fee of $225.
